Registered Clinic Participants Instructions

Dear Training Clinic Participant, On site check-in for the Wyonegonic Training Clinics will include a brief health assessment. Many of you will be traveling from a variety of places in the USA and abroad. We need to be vigilant about the health of everyone in residence. We will not be admitting any participant who has flu like symptoms that include fever over 100 degrees accompanied by a cough. As a reminder, a copy of your health form is still a prerequisite for attending a Training Clinic at Wyonegonic.  If  you are turned away for medical reasons, a full refund will be provided. Health Form for Participants

All participants are required to provide Wyonegonic Camps with a health form signed by a physician, within 24 months of the date of your clinic. A current health history must also be documented with your signature. A copy of your camp’s form will suffice. You can send a copy of your form to the camp office prior to your arrival. You will not be able to begin the clinic without producing a current health form! If you do not have a blank health form, click here to download one.
